What We Collect

When you submit our contact or appointment request form, we collect the information you provide: your name, phone number, email address, appointment address, service requested, preferred date and time, and any comments you include.

How We Use It

Submissions are emailed directly to Paula and Michael Hoffman so we can respond to your request and confirm an appointment. We do not sell, rent, or share your information with third parties, and we do not use it for marketing.

What Not to Submit

Please do not include Social Security numbers, complete financial account numbers, medical records, unredacted identification, or other sensitive document contents in the contact form.

Notarized Documents

Information you provide during an actual notarization (such as identification shown at the appointment) is handled according to Alabama notary recordkeeping requirements, separate from this website's contact form.
